About the Founder
Candice Elam, DNP, is a Family Nurse Practitioner with over a decade of diverse healthcare experience. She currently works full-time as a primary care provider at a Federally Qualified Health Center, where she provides comprehensive care spanning prenatal, pediatric, adult, and geriatric populations. Candice holds board certifications as an HIV Specialist through the American Academy of HIV Medicine and as a Diabetes Care and Education Specialist through the Association of Diabetes Care & Education Specialists.
Her career has included roles in emergency care, clinical trials research, and community health, giving her a unique perspective on the challenges faced by primary care providers. Recognizing the common struggles with documentation, delegation, time management, and communication, Candice founded SignTheChart to offer practical, effective solutions. Through online courses, Chart Smart Kits, and consulting, she distills the expertise and efficiency strategies she’s developed to help healthcare professionals and healthcare organizations optimize their workflows and reclaim their time.
